Selecting anticoagulant therapy in patients with cirrhosis

چکیده
Patients with liver diseases have a higher risk of hemostatic disorders. Meanwhile, despite well-established opinion on low thromboses in cirrhosis, thromboembolic events cannot be totally ruled out these patients. Increased risks thrombotic and venous thromboembolism cirrhosis are accounted for by external (immobilization, surgeries, infections, comorbidities) internal factors (reduced synthesis anticoagulants, increased levels estrogens). Practically, the selection anticoagulant therapy is difficult because divergent disorders requires continuous careful monitoring hemostasis. In 2019, American Gastroenterological Association (AGA) released guidelines patients pulmonary embolism. We describe patient A management strategy was tailored to AGA guidelines, Russian Society Liver Research Fibrosis Cirrhosis Complication Diagnosis Treatment guidelines. This example successful therapy, multidisciplinary approach, dynamic evaluation all available methods hemostasis control will useful practitioners. KEYWORDS: hemostasis, thrombosis, hemorrhage, thromboembolism, embolism, thromboelastometry, anticoagulant, low-molecular-weight heparin. Antioxidant Levels in the Saliva of Patients with Cirrhosis Compared with Patients Post-Transplantation
The aim of this study was to compare antioxidant levels of saliva in patients with cirrhosis and liver transplantation. This study was carried out on 45 subjects referred to the Gastroenterohepatology Clinic of Dicle University Medical Faculty. These subjects were composed of cirrhosis, transplanted patients, and control group with no liver problems. The saliva samples were taken in the morning...
متن کاملOral surgery in patients on anticoagulant therapy.
OBJECTIVE Surgery is the main oral healthcare hazard to the patient with a bleeding tendency, which is mostly caused by the use of anticoagulants. The traditional management entails the interruption of anticoagulant therapy for dental surgery to prevent hemorrhage. However, this practice may increase the risk of a potentially life-threatening thromboembolism. Because this issue is still controv...
متن کاملAnticoagulant therapy in Japanese patients with mechanical mitral valves.
There are no guidelines for the optimal therapeutic range of anticoagulant therapy in Japanese patients with mechanical heart valves. A total of 214 patients were followed retrospectively after mitral mechanical valve replacement (mean duration of follow-up, 4.8 years; total duration of follow-up, 1,027 patient-years). The target range of the international normalized ratio (INR) for oral antico...
متن کاملOptimal oral anticoagulant therapy in patients with mechanical heart valves.
BACKGROUND The optimal intensity of oral anticoagulant therapy for patients with mechanical heart valves (i.e., the level at which thromboembolic complications are effectively prevented without excessive bleeding) is not known. We attempted to determine the optimal intensity by calculating the incidence of both complications at different levels of anticoagulation. METHODS Data were collected ...
متن کاملArgatroban anticoagulant therapy in patients with heparin-induced thrombocytopenia.
BACKGROUND Heparin-induced thrombocytopenia (HIT) is an immune-mediated syndrome caused by heparin. Complications range from thrombocytopenia to thrombocytopenia with thrombosis. We report a prospective, historical- controlled study evaluating the efficacy and safety of argatroban, a direct thrombin inhibitor, as anticoagulant therapy in patients with HIT or HIT with thrombosis syndrome (HITTS)...
